Case managers are trained professionals adept at performing assessments and recommending appropriate services or placement for people who are elderly or disabled. Case managers typically meet with clients in their homes. They assess and then recommend and try to find services that will maintain that client's living situation within the community. The goal is to help each person remain in the least restrictive setting.
