Intake Process:	Notify one month prior to admission if coming from a private home by calling the Pre-Admissions Screening Coordinator at 574-2842644. Facilities notify us directly of all hospital transfers.

The PAS program is a state program which mandates everyone who enters a nursing home or applies for admission at a nursing home be screened. This screening process entails reviewing the client's physical needs to ensue that person has enough physical impairments to warrant nursing home placement.
